service can order their O-riginals toppings through the Customer
Relations Associate who takes their regular cookie dough

Foodservice operators who purchase their Otis Spunkmeyer
cookie dough through foodservice distributors can purchase their
O-riginals toppings directly from Otis Spunkmeyer via the attached
O-riginals Order Form. All customers ordering
with the order form will need to complete either a credit
application or a credit card authorization form before
the order can be processed. All billable goods require that
one of these forms be processed.
Once you have submitted one of the above, you can send
your O-riginals Order Form for processing. Instructions
for order placement and contact information are included on
the order form.
